We like to call your attention regarding your tardiness for the last ten (10) days. It had appeared in our time monitoring system (TMS) that you constantly arrived at 10:15am since August 12, 2003. Please be reminded that the official working hours starts at exactly 9:00am and ends at 6:00pm.

This will be a permanent record on your 201 file. As stated in the company policy hand book on page 8 section 10.1.3 regarding tardiness, the next offense will be five (5) days suspension.
